Are you using it idiomatically or literally?

Literally, it can mean that all the phlegm or saliva has been completely cleared from the throat by coughing.

Idiomatically, I think I'd heard it being used once or twice jokingly to refer to something being "well said". But it's definitely not something commonly used in English.
