It depends on what is meant.

I don't understand what you are explaining to me. = Hindi ko naiiintindihan ang ipinapaliwanag mo sa akin. (too complicated to understand)

I don't understand Chinese. = Hindi ako nakakaintindi ng Chinese. (totally "foreign" for me)

I don't understand what her problem is. = Hindi ko maintindihan kung ano ang problema niya. (no idea about the cause of her problem)
