”to make it out alive” means that someone continues to live after a situation where it is possible to die. We usually use this expression when the person is still alive but has either suffered a physical injury or psychological trauma.


e.g. ”There was a terrible car crash. He made it out alive, although he had to stay in hospital for 2 weeks.” = ”It was possible to die in that car crash. He survived, but with injuries which required treatment in hospital for 2 weeks.”
