To make this statement grammatically correct, you would need to make ”aggravated” passive:

”A cough that he has had occasionally for about 6 months has lately BEEN aggravated.”

No comma because the ”6 months” is a defining statement regarding the specific cough he has had.

Medically more correct would be to say:

”A chronic cough he has suffered for approximately 6 months has recently been aggravated.”
