    • My experience

    • Last year, my five months internship at Reims University Hospital in the Geriatric Post- Emergency and Clinical Pharmacy departments allowed me to put into practice and reinforce my knowledge.
    • I conducted hundreds of patients interviewed about their diseases, care pathway, medicines, with the aim to understand and optimize their treatments.
    • Completed with my prior internships in pharmacy, I understood better the healthcare system and the reality on the ground including the difficulties related to staffs or patients.
    • I gained important professional experience and optimized my knowledge of patient care (prise en charge patient).
    • It also gave me a better understanding of the functioning and organization of the hospital, and how it is managed.
    • In clinical pharmacy, I was soon given the responsibility of carrying out interviews for transplant patients.
  • Immunosuppressive treatments have to be taken for life and are therefore restrictive.
    • Failure to adhere to or comply with post-transplant drug treatments is a major problem that can lead to graft rejection and patient death.
    • It is therefore essential to have a discussion with the patient to explain the importance of their treatment and thus encourage compliance.
    • I also ensured continuity of care by contacting the patient’s (retail) pharmacy to remind them that the drugs cannot be substituted.
    • The patient must also be careful about self-medication, so it is important that the pharmacy is aware that the patient has had a transplant.
    • This responsibility gave me more confidence in myself and my medical knowledge, and allowed me to be more autonomous.
    • I also learned how to communicate in a relevant way to patients so that they can better understand their condition /medicines.
