An urgent referral letter
- I am writing to refer Ms Styles, to request an urgent assessment and further management for her recently diagnosed pericarditis.
- Regarding Ms Styles social and medical history, she lives alone.
- She has had hypertension and diabetes mellitus since 2012 and 2009 respectively.
- She has been taking medications such as carpinol, metformin and glipizide.
- She has been exhibiting symptoms of depression over the last 3 years.
- Ms Styles was admitted to the green valley hospital on 07/04/2018 with the complaints of shortness of breath and tiredness.
- During hospitalization her lab results were erythrocyte sedimentation rate 24, creatinine-2.0 and decreased platelets.
- Type B influenza was noticed in her throat swab and her echocardiogram report revealed type B influenza plus pericarditis.
- She was treated with antibiotics.
- She was discharged on 09/04/2018 and advised further follow up home visits.
- On 14/04/2019, when I visited Ms Styles at her home she explained she was on self-care after her discharge.
- During examination, she complained chest pain, tiredness and fatigue.
- Her vitals were heart rate-122, respiratory rate-22, blood pressure was -180/90 and she has mild temperature.
- However, her chest pain was declined while sitting.
- Her clinical signs and symptoms are suggestive of pericarditis.
