”What am I to do?” Is this a phrase that mostly used in a spoken language or rather it is a book one?

Most likely also British English rather than American English.

”What am I to do?” is quite a formal phrase. It would have been used in older literature, spoken by an elegant lady. If spoken in today’s modern culture, people would understand you but might consider it odd. I would use ’What can I do?’ or ’What should I do?’
