Usually, the only way to distinguish those kinds of words when you hear them, called homonyms, is through the use of context clues. This means figuring out which word is being used based on the sentence as a whole. In written text, obviously you can plainly see which word is being used.

This comes with practise. Here is a preposition, hear is a verb. This demonstrates how important grammar study is. Only with practise can you become better at identifying the context. Of course learning vocab often in small doses speeds this process up.
