• RE VISA APPLICATION March 21st 2022 Greetings My name is Willington Diaz Ortega with identification number 79835811 from Bogotá I am the administrative coordinator at the company Movements and Transports in Bogotá I have worked there since 2015 in differents constructions with a long-term contract.

• I plan to visit Canadá for two reasons: The first one I have an eye surgery scheduled for April 30 of this year at the clinic of Toronto.

• The second one is I want to visit to my friend Jhonnatan who lives in Toronto.

• Because he has offered me his house to recover from my surgery.

• I plan to stay a month because I have to get back to Colombia to fullfil my work responsabilities.
