Freekarol is right. "Doktor" come from latin (lat. doctor) and "lékař" is slavic/celtic origin. Lék = drug. "Doctor is probaly a bit universal.

Both is used in Czech commonly. Lékař might be maybe little bit more formal. But that´s subjective.

doktor - 1. držitel některých vysokoškolských titulů
2. (v obecném jazyce) lékař

They means the same thing. We say both interchangeably.
