1. I might forget you ; Maybe I will forget you or Maybe I forget you. It depends on the context.
2. This is not what you might hear in every day speech, but in a song it can mean " maybe you will forget me". In standard speech you would say: شاید فراموشم کنی .

The first sentence means"'Perhaps I forgot you".
the second sentence means "Perhaps you forgot me." the second isn't useful in slang but you can find in poets and songs like: "شاید فراموشت شدم، شاید دلت تنگه برام" means:"Perhaps you forgot me,Perhaps you missed me"

Vice versa! I think you should exchange the numbers. The first one says: "Maybe I vanished you from my mind" and the second one says: "Maybe you have forgotten me".

The first wording is correct, but the second wording is wrong .
شاید فراموشت کردم صحیح است .
ولی شاید فراموشت شدم غلط است . باید میگفتی شاید تو مرا فراموش کردی این که شما فرمودی به زبان عامیانه درون مردمی است نه زبان رسمی و کتابی
