"Ce l'ho fatta" is used when you have been able to do something. E.g. "Ce l'ho fatta, ho superato l'esame" is "I did it, I passed the exam"

L'ho fatto/a is used when you have done/made something (note that italian does not distinguish between "to do" adn "to make", it always uses "fare"). It means "I made/did it"
