In the most simple of terms it means "to fall", but metaphorically it can symbolize "failure", i.e., "to fail" miserably @ something -- an activity, etc.

As exRanger and kso89 said. I interpret it also as an insult similar to, "Go [and] jump off a cliff", expressing one's dislike for someone.
